Extended Access - Hub Appointments

IMPROVING ACCESS TO PRIMARY CARE
Appointments are available for evenings and weekends at one of the six satellite hubs around the city.
If it is deemed appropriate, you may be offered an appointment at one of six GP access centre's across Sheffield 6pm-10pm weekdays and 10am-6pm weekends.
GP appointments can be booked direct by the patient's practice or by 111.
The services available include: urgent appointments with a GP.
Practice nurse/HCA appointments for the following routine services: blood pressure checks, removal of stitches, dressing changes (patients must bring their new dressings with them to the appointment), asthma reviews/checks, diabetes checks/reviews, ear syringing, smears, blood tests.
Physiotherapy appointments are also available.

Flowers Practice - 6-10pm Monday - Friday
Crookes Practice - 6-10 pm Monday – Friday 10-6 pm Saturday, 10-2 Sunday
Woodhouse Health Centre - 6-10 Monday - Friday 10-6 Saturday, 10-2 Sunday
Palgrave Surgery - 6-10 Monday - Friday 10-6 Saturday - Sunday
Sloan Medical Centre - 6-10 Monday - Friday 10-6 Saturday - Sunday
Chapelgreen Surgery - 6-10 Monday - Friday
