How To Register

Please ask for a registration form from the reception desk.
Bring the completed form to reception with your NHS medical card. This gives us fast access to your old records. However, if you have lost your card - don't worry!  Once you have been accepted as a patient, your medical records will be transferred to us and a new medical card will be sent to your home address. You do not need to tell your old practice that you are moving unless you wish to say goodbye.
It is helpful if you can bring proof of identity (e.g. passport, driving license) and address (e.g. utility bill). You will not be refused registration if you do not have these details, we only request them to allow you access to online services and to ensure you are living within our practice boundary.
EEC residents are entitled to NHS treatment.
Non-EEC citizens must have a visa that allows residency for more than 6 months. If you are a non-EEC citizen on holiday, or staying with a relative, for six months or less, you will need to register as a private patient at another surgery/service nearby.  We do not register private patients. You will be charged for services as a private patient and you are not entitled to free NHS prescriptions.  
Emergency services are provided to everyone, irrespective of immigration status.
New Patient Check
At registration you will be offered a new patient check with our Health Care Assistant. She will perform a simple health check and assess how best we can help you. Please bring any medication or inhalers with you. This appointment helps us make sure your records are correct,  and check that all your routine health checks (e.g. blood pressure, smear test and vaccinations are up to date. Please try to attend this appointment.
Catchment Area
We cover parts of Sheffield 4, 5 & 9. We do not accept patients outside the area, due to the difficulties in providing home visits in emergencies or during episodes of prolonged illness. Please contact the surgery on 0114 2435504 to check if your address is within our catchment boundary.

Patients who have been removed from the list for aggressive or abusive behaviour will not be re-registered.
